The government of Indonesia is providing free Blangko Kartu Keluarga (BKK) to layanan layanan pemerintah, which can be downloaded and printed from the web page. The card is used for family member identification and serves as a password for accessing social service benefits such as health care, education, employment, and community programs. More than 16 million BKK cards have been issued since the program was launched in 2014. This article provides information about how an individual can obtain a Blangko Kartu Keluarga (BKK) by downloading it from the website of the government of Indonesia. The BKK is a popular identification card with detailed information such as family members (mother, father, wife, husband, and children), address of the household. This is aimed at facilitating government services such as health care and education for family members. For each citizen age 17 and above, a Smart Card named e-KTP was also issued and used for identification and authentication purposes. However it cannot be used for obtaining social services or benefits from the government. First of all, an individual must access the website http://www.bkk-online.go.id/ to download the application forms. Once the application form is completed, the Indonesian electronic identity card (e-KTP) application form has to be printed and signed. The e-KTP application form (which is designed as an electronic form, without paper copy) contains basic information about the card owner. Here are the fields that can be filled in by the individual applicant: Name of the institution Sample name of the institution Office address Phone number Office address including post code Sample address of where to apply for e-KTP card will be sent to by email once processing is completed. If an individual is a dependent of a registered parent, the custodian must fill in the information below: Name of the dependent Sample name Dependent's age and gender Dependent's type: dependent or spouse/partner Dependent's national identification number (KTP) or any other identification document that can prove that he or she is the dependent. This field is not mandatory for dependent children of non-Indonesian citizen. If an individual needs to apply for social services such as family health care, social assistance, and education benefits, he must download and apply for BKK card within four weeks from receiving notification of eligibility. To apply for BKK, the individual must present family members (mother, father, wife, husband, and children) or copies of their documents. A photo or video of each member is required for verification purposes. Indonesian citizens who are not able to read and write can get an official certificate of illiteracy (K3) at the agency that issues the BKK. A certificate (K3) is issued to an individual who has proven that he/she cannot write his/her own name because it is written in national language. The certificate enables him/her to get a BKK card using their signatures. eccc085e13
